Care Plan Agreement

By submitting this form you confirm that:

• You have read and agree to the ClimateWorks Care Plan Terms and Conditions.

• You understand that the Care Plan is a 12 month agreement which renews automatically unless cancelled.

• Payments will be collected monthly by Direct Debit.

• The plan includes one annual service and up to two breakdown visits per year.

• Replacement parts and labour beyond the first hour are chargeable.

Immediate Service Request

By selecting this option you request that ClimateWorks begins providing Care Plan services immediately.

You acknowledge that:

• You still have a 14 day cooling off period under UK consumer law.

• If you request services within this period and then cancel, ClimateWorks may charge for services already carried out.
