Cooling Off Period and Care Plan Agreement

By submitting this form you confirm that:

• You have read and agree to the ClimateWorks Care Plan Terms and Conditions

• You understand that the Care Plan is a minimum 12 month subscription which automatically renews unless cancelled

• Payments will be collected monthly by Direct Debit

• The plan includes one annual service and up to 1 breakdown visit per year

• Replacement parts and labour beyond the first hour are chargeable

• The Care Plan is a maintenance agreement and not an insurance policy

Service Timing Acknowledgement

• You understand that the Care Plan does not include an immediate service

• The included annual service becomes due after 12 months from the start date

• If your system is due a service within the first 30 days of joining, this will be charged at the standard rate

• All other Care Plan benefits become active after a 30 day waiting period

Payment and Cancellation Acknowledgement

• You understand that if you cancel the Care Plan, no refunds will be provided

• Any remaining payments within the active 12 month term remain payable in full

Cooling Off Period and Immediate Service Request

By selecting this option you request that ClimateWorks begins providing Care Plan services after the standard 30 day waiting period.

You acknowledge that:

• You still have a 14 day cooling off period under UK consumer law

• If you request chargeable services within this period and then cancel, ClimateWorks may charge for services already carried out
